As a person receiving services or a family member, it is your role to speak up when it comes to defining quality services. You have the responsibility to tell someone when you are not satisfied with services and it is your responsibility to define what your expectations are from the providers who provide those services to you or your family member. If you feel your requests or concerns have not been addressed to your satisfaction, you can contact your المكتب الإقليمي or contact DBHDD Constituent Services.
The Georgia Collaborative ASO will also need your help in evaluating the quality of services. If you are randomly selected to participate in any of the review processes, Person Centered Review, Quality Enhancement Provider Review or Quality Technical Assistance Consultation, a representative from the Georgia Collaborative ASO will contact you to see if you would like to participate. It is voluntary and will not impact your services being received. If you would like to participate, we will interview you and any family member(s) or others whom you would like present. We will talk with you about the quality of the services you receive regarding six (6) key areas of the service system: Health, Safety, Rights, Choice, Community and Person Centered Practices. We will use your feedback to help improve services by providing recommendations and ideas to better support your needs, dreams and goals in life. We will also combine your feedback with others who participate and provide recommendations to improve the service delivery system for everyone receiving services in the state of Georgia.
