Authorizations are determined through medical necessity and apply only to a specific provider and service(s). An authorization does NOT guarantee payment of benefits for these services. Payment depends on the individual’s plan on the date(s) the services are provided. Coverage is subject to all limits and exclusions outlined by DBHDD and/or summary plan description. Examples of coverage limits/exclusions include copayment charges, deductibles and coinsurance, annual, lifetime or episodic maximums and pre-existing conditions.
