Payment depends on the individual’s plan on the date(s) the services are provided. Coverage is subject to all limits and exclusions outlined by DBHDD and/or summary plan description. Examples of coverage limits/exclusions include copayment charges, deductibles and coinsurance, annual, lifetime or episodic maximums and pre-existing conditions.
